Job Summary
AlliedUP is seeking a Behavioral Health Access Coordinator to serve as the clinical front door for Behavioral Health Services within a collaborative Federally Qualified Health Center (FQHC). This role is responsible for conducting initial behavioral health screenings, triage, and care coordination to ensure patients are connected to the appropriate level of behavioral health, primary care, or community-based services.
Working as part of an interdisciplinary healthcare team, the Behavioral Health Access Coordinator supports individuals experiencing depression, anxiety, trauma, substance use disorders, chronic illness-related stress, and other behavioral health concerns. The ideal candidate is passionate about improving access to care, coordinating services across multiple programs, and ensuring patients receive timely, person-centered support from their first point of contact through successful engagement in treatment.
Key Responsibilities
Behavioral Health Screening & Triage
- Serve as the primary access point for referrals from Primary Care, community partners, walk-ins, self-referrals, AB-109, BHSA, and other referral sources
- Conduct initial behavioral health screenings using PHQ-2/9, GAD-7, substance use screenings, suicide risk assessments, brief mental status assessments, and clinical triage
- Manage behavioral health access queues, monitor waitlists, prioritize urgent referrals, and escalate high-risk patients for immediate intervention
- Determine the appropriate level of care based on patient needs and clinical presentation
- Coordinate referrals to Integrated Behavioral Health (IBH), Specialty Mental Health (SMH), Psychiatry, Substance Use Disorder (SUD) services, Primary Care, Nursing, Behavioral Health leadership, and other internal programs
- Facilitate warm handoffs between providers and support timely program enrollment
- Coordinate Social Determinants of Health (SDOH) needs through Enhanced Care Management (ECM), Community Health Workers, Community Supports, and appropriate community-based resources
- Connect patients with housing, food, transportation, employment, legal assistance, benefits, and other supportive services when appropriate
- Participate in interdisciplinary care team huddles, quality improvement initiatives, Joint Commission readiness activities, and population health efforts
- Close the referral loop by confirming appointments, documenting outcomes, and ensuring patients successfully connect to recommended services
- Conduct outreach and re-engagement efforts for patients who miss appointments or require additional follow-up
- Promote timely access to behavioral health services while supporting a positive patient experience throughout the care journey
- Document screenings, assessments, referrals, outreach activities, care coordination, and patient interactions accurately within Epic
- Ensure compliance with HIPAA, 42 CFR Part 2, organizational policies, and documentation standards
- Track and report key performance metrics including referral volume, time to triage, time to first appointment, referral completion rates, no-show rates, warm handoff rates, and closed-loop referral completion
- Bachelor's degree in Social Work, Psychology, Human Services, Public Health, or a related field
- Minimum of two years of experience in behavioral health, integrated care, care coordination, case management, community health, or an FQHC setting
- Experience conducting behavioral health screenings and coordinating patient care across multidisciplinary teams
- Strong interpersonal, communication, organizational, and problem-solving skills
- Ability to effectively engage individuals from diverse cultural and socioeconomic backgrounds
- Experience documenting patient encounters within an Electronic Health Record (EHR)
- Knowledge of trauma-informed care and motivational interviewing
- Experience utilizing behavioral health screening tools including PHQ-2/9, GAD-7, substance use screenings, and suicide risk assessments
- Familiarity with CalAIM, Enhanced Care Management (ECM), Community Supports, and community resource navigation
- Experience working within an integrated behavioral health or Federally Qualified Health Center (FQHC) environment
- Access or Epic EHR experience strongly preferred
- Knowledge of Social Determinants of Health (SDOH) and community-based care coordination
