Community Health Worker (CHW)
Location: San Jose, CA (locations in East and South San Jose) 
Pay: $24.00–$27 per hour
Job Type: Long Term Contract, Full-Time

Shift: Monday – Friday 8:00 – 5:00 and 10-7 one weekday per clinic hours.

AlliedUP Co-Op is seeking compassionate, community-focused Community Health Workers (CHWs) to help individuals and families access healthcare, navigate community resources, and improve health outcomes at our client – a mission-driven, federally qualified health center (FQHC) dedicated to providing high-quality, comprehensive healthcare and supportive services to diverse communities throughout the Bay Area. As a trusted member of the care team, you’ll build relationships with patients, provide health education, coordinate services, and connect individuals to medical care, health insurance, and essential community resources.

Depending on your certifications you may be a Community Health Worker I (CHW Certification) or II (CHW and CEC Certification)

What You’ll Do

Responsibilities may include:
  • Build trusted relationships with patients and families
  • Conduct community outreach and participate in health fairs and community events
  • Perform Social Determinants of Health (SDOH) screenings and other patient assessments
  • Document patient encounters in the electronic health record (EHR)
  • Schedule appointments and coordinate interpreter and transportation services
  • Follow up with patients after missed appointments
  • Connect patients to healthcare services, behavioral health, specialty care, and community resources
  • Assist patients with enrollment in public benefits and health insurance programs
  • Coordinate care with providers, case managers, and interdisciplinary care teams
  • Educate patients on topics including nutrition, physical activity, chronic disease prevention, hygiene, sexual health, and substance use prevention
  • Support patients experiencing barriers related to housing, food insecurity, transportation, and financial assistance
  • Assist families with advocacy, crisis intervention, and problem-solving
  • Participate in care team meetings, trainings, and case conferences
  • Maintain accurate documentation and assist with grant reporting and program evaluation activities
  • Travel to clinics, schools, mobile health units, community events, homeless encampments, warming centers, and patient homes as assigned
  • Work occasional evenings and weekends to support community outreach
Qualifications

Required
  • High school diploma or equivalent
  • Community Health Worker (CHW) Certification required
  • Strong communication and relationship-building skills
  • Comfortable working with diverse communities
  • Basic computer and Microsoft Office skills
  • Valid California driver’s license, reliable transportation, and the ability to travel between clinic and community locations throughout the service area
  • Ability to work occasional evenings and weekends
  • Bilingual skills are highly preferred. Languages include Spanish, Mandarin, Cantonese, Hindi, Punjabi.
Preferred
  • Certified Enrollment Counselor (CEC) certification, or the ability to obtain certification within the first 90 days of employment.
  • Experience with patient navigation, care coordination, case management, health education, or community outreach
  • Community Health Worker experience in healthcare, public health, social services, or community-based organizations preferred
