Resident Care Coordinator

Location: San Jose, CA
Pay: $24.00 per hour
Employment Type: Contract-to-Hire
Schedule: Sunday–Thursday, 8:00 AM–5:30 PM

About the Opportunity

AlliedUP is seeking an experienced and compassionate Resident Care Coordinator to join a premier Continuing Care Retirement Community (CCRC) in San Jose.

In this role, you'll help coordinate resident care across Assisted Living, Memory Care, and Independent Living while working closely with residents, families, and the clinical team. This is an excellent opportunity for someone who enjoys leading by example, supporting caregivers, and ensuring residents receive exceptional, person-centered care.

Please note:
This position requires flexibility to cover Med Tech shifts as needed.

Responsibilities

  • Coordinate resident care services and ensure individualized care plans are followed.
  • Participate in resident assessments and service planning.
  • Monitor resident health and communicate changes to nursing and wellness leadership.
  • Collaborate with caregivers, nursing staff, and other interdisciplinary team members.
  • Serve as a resource for residents and families by providing updates and addressing care-related questions.
  • Assist with scheduling and staffing to ensure appropriate resident coverage.
  • Document resident care accurately in electronic documentation systems.
  • Provide guidance and support to care staff on resident care procedures and documentation.
  • Assist with direct resident care as needed.
  • Cover Med Tech shifts when necessary to support the community.
  • Maintain compliance with infection control, safety, and regulatory standards.

Qualifications

  • Previous experience in Assisted Living, Memory Care, Long-Term Care, or a related healthcare setting.
  • Strong leadership, communication, and organizational skills.
  • Experience coordinating resident care and collaborating with interdisciplinary teams.
  • Familiarity with care planning and electronic documentation systems preferred.
  • Knowledge of person-centered care and senior living best practices.
  • Ability to prioritize multiple responsibilities in a fast-paced environment.
  • Professional, dependable, and detail-oriented.
  • Willingness to provide hands-on support and cover Med Tech shifts as needed.
