Bring Skilled Nursing Care to Patients at Home

AlliedUP Co-op is seeking an experienced Licensed Vocational Nurse (LVN) to provide skilled Home Health nursing services to patients throughout Santa Cruz County.

We are seeking a Full-Time LVN to join their Home Health team. In this role, you'll provide skilled nursing care to patients throughout Santa Cruz County/Mid-County including Santa Cruz, Aptos, Capitola, Soquel, Watsonville, Aromas, and neighboring areas.

Working under the supervision of a Registered Nurse, you'll provide direct patient care in the home, follow individualized plans of care, educate patients and caregivers, and collaborate with an interdisciplinary Home Health team.

What You'll Do

Provide skilled nursing care in patients' homes according to the established plan of care
Perform treatments and nursing services within LVN scope of practice
Monitor patient condition and response to treatments and medications
Provide pain and symptom management as directed
Educate patients and caregivers on diagnoses, medications, treatments, and self-care
Complete accurate and timely clinical documentation
Report changes in patient condition to the supervising RN
Coordinate care with RNs, Home Health Aides, PT, OT, ST, MSW, and other care team members
Use medical equipment and supplies safely and efficiently

What We're Looking For

Graduate of an accredited Vocational Nursing program
Current California LVN license
1–2+ years of nursing experience
Direct Home Health nursing experience required
Current CPR certification
Strong clinical, communication, documentation, and organizational skills
Ability to work independently in a field-based environment
Valid California driver's license, reliable insured vehicle, and ability to travel to patient homes throughout Santa Cruz County
Understanding of the Home Health model of care
