Community Health Worker (CHW)

Pay: $24.00–$27.00/hour
Job Type: Full-Time, Long-Term Contract
Location: On-Site Fremont or San Jose
Schedule: Monday–Friday; shifts may include 8 AM–5 PM, 10 AM–7 PM, or 11 AM–8 PM based on clinic hours. Occasional weekends.

About the Role

AlliedUP Co-Op is hiring Community Health Workers (CHWs) to support a mission-driven Federally Qualified Health Center (FQHC) serving communities throughout the Bay Area.

You'll help patients access healthcare, community resources, health coverage, and public benefits.

Important: Previous hands-on experience enrolling individuals in Covered California health plans and Medi-Cal is required.

What You'll Do

  • Assist patients with Covered California, Medi-Cal, CalFresh, and other public benefit programs
  • Explain eligibility, enrollment requirements, and required documentation
  • Conduct outreach and Social Determinants of Health (SDOH) screenings
  • Connect patients to healthcare, behavioral health, and community resources
  • Support needs related to food, housing, transportation, and financial assistance
  • Schedule appointments and coordinate patient services
  • Provide health education and patient navigation
  • Document patient interactions in the EHR
  • Coordinate with providers, case managers, and care teams
  • Participate in community outreach events
  • Travel between clinics and community locations as needed

Required Qualifications

  • High school diploma or equivalent
  • Community Health Worker (CHW) program Certificate of Completion
  • Experience enrolling individuals in Covered California and Medi-Cal
  • Experience with CalFresh and/or other public benefit programs
  • Strong communication, outreach, and relationship-building skills
  • Basic computer skills
  • Valid California driver's license and reliable transportation
  • Ability to travel between assigned locations

Bilingual highly preferred: Spanish, Mandarin, Cantonese, Hindi, or Punjabi.

Preferred Experience

  • FQHC or community health experience
  • Patient navigation, care coordination, case management, or outreach
  • Electronic Health Record (EHR) systems
