Location: On-Site in Oakland, CA

Pay: $27.00

Help Improve Birth Outcomes for Black Families
AlliedUP Co-op is hiring a Community Health Advocate / Group Facilitator to support the Black Infant Health (BIH) Program with one of Alameda County’s leading Federally Qualified Health Centers.
In this role, you’ll facilitate prenatal and postpartum support groups, provide one-on-one case management, connect participants with community resources, and help improve maternal and infant health outcomes through culturally responsive care.

What You’ll Do

  • Co-facilitate prenatal and postpartum BIH group sessions in accordance with program curriculum 
  • Build trusting relationships with Black pregnant and parenting participants
  • Provide monthly case management and individualized support
  • Develop Life Plans, Birth Plans, and connect participants to community resources
  • Coordinate care with healthcare providers and community organizations
  • Document services in ETO and Epic while maintaining HIPAA compliance
  • Participate in community outreach, trainings, and program meetings
Work Schedule
  • Full-time
  • Occasional evenings and weekends
  • Travel throughout West Oakland, Emeryville, North Oakland, and South Berkeley (mileage reimbursement provided)
Qualifications
Required
  • Bachelor’s degree in Social Work, Public Health, Health Education, Human Services, Nursing, Psychology, or a related field
  • 1+ year of experience in community health, case management, maternal health, or social services
  • Experience working with Black/African American communities using culturally responsive and trauma-informed practices
  • Strong communication, facilitation, and organizational skills
  • Valid California driver’s license and ability to travel throughout Alameda County
Preferred
  • Experience facilitating groups or workshops
  • Maternal and child health or FQHC experience
  • Experience with Epic or ETO
