Registered Nurse (RN) – Home Health

Full-Time | Direct Hire
Location: Salinas and surrounding communities, including Salinas, South Salinas, Prunedale, Aromas, Castroville, East Garrison, Corral de Tierra, Spreckels, and neighboring areas.

Schedule: Rotating on-call responsibilities, as assigned.
Compensation: Per Visit and Salaried available ranging from $75-$90 per hour.

They are seeking a Full-Time Registered Nurse (RN) to join their Home Health team. In this role, you'll provide skilled nursing care to patients throughout greater Salinas area including Salinas, South Salinas, Prunedale, Aromas, Castroville, East Garrison, Corral de Tierra, Spreckels, and neighboring areas. Responsibilities

  • Conduct comprehensive patient assessments and develop individualized plans of care.
  • Provide skilled nursing services, including medication administration, wound care, disease management, and physician-ordered treatments.
  • Monitor patient progress and evaluate patient responses to treatment, adjusting care plans as appropriate.
  • Educate patients and caregivers on diagnoses, medications, treatment plans, disease management, and self-care techniques.
  • Collaborate with physicians, therapists, case managers, and other healthcare professionals to coordinate quality patient care.
  • Complete timely, accurate, and compliant clinical documentation.
  • Recognize changes in patient condition and communicate effectively with the interdisciplinary care team.
  • Provide compassionate support, education, and advocacy for patients and their families.
  • Manage a consistent weekly caseload of approximately 25 patients, allowing you to build meaningful relationships while delivering high-quality, patient-centered care. Additional compensation opportunities are available for clinicians who choose to take on visits beyond their standard caseload
  • Participate in interdisciplinary team meetings and quality improvement initiatives.

Requirements

  • Current, unrestricted California Registered Nurse (RN) license.
  • Minimum one (1) year of professional RN experience required.
  • Home Health nursing experience preferred.
  • Current BLS/CPR certification required.
  • Valid California driver's license, reliable transportation, and current auto insurance.
  • Ability to travel throughout the assigned service area.
  • Strong clinical assessment, critical thinking, and care planning skills.
  • Excellent communication, interpersonal, and organizational skills.
  • Experience with electronic medical records (EMR) documentation preferred.
  • Experience with wound care, IV therapy, and OASIS documentation is a plus.
  • Ability to work independently while collaborating effectively with an interdisciplinary care team.
