Registered Nurse (RN) – Indio, CA
Salary: $110,000 – $130,000 (Based on Experience)
Schedule: Mon 8:30am–5:00pm | Tue 10:00am–7:00pm | Thu–Fri 8:30am–5:00pm | Sat 6:30am–3:00pm
Location: Indio, CA | Travel Required (Southern California)
Our client, a leading network of community-based clinics across Southern California, provides integrated medical, dental, mental health, and substance use services. We are currently seeking a mission-driven Registered Nurse (RN) to support their newly launched clinic in Indio, CA.
This is a rewarding opportunity for a dedicated RN who is passionate about community health and public service. You’ll play a leadership role, managing a multidisciplinary team of LVNs and Lead Care Managers, helping shape and expand the delivery of essential health programs such as CalAIM Enhanced Care Management and Community Supports.
Key Responsibilities

Provide clinical and programmatic leadership to a team of 11+ staff, including oversight of remote clinic operations.

Supervise day-to-day program delivery, including outreach, health education, care coordination, and patient advocacy.

Lead the planning, development, implementation, and evaluation of care management programs and related services.

Coordinate Multidisciplinary Team Meetings and support collaboration across medical providers, housing specialists, and other partners.

Oversee documentation and data entry in eClinicalWorks, ensuring accuracy and compliance with contracts and grants.

Monitor performance metrics, evaluate outcomes, and ensure adherence to quality standards.

Develop MOUs and maintain partnerships with external agencies.

Prepare and deliver internal reports, training sessions, and presentations as needed.

Support integration of care management programs into the clinic’s clinical workflow and broader strategic objectives.

Participate in relevant local, state, and national conferences.
Required Qualifications

Associate Degree in Nursing (ADN) required.

Active RN license in California required.

Minimum 3–5 years of nursing experience, with at least 1 year in a supervisory or care management role.

Current BLS Certification (American Heart Association).

Strong working knowledge of health data systems, community health evaluations, and clinical workflows.

Excellent communication and leadership skills with demonstrated ability to manage diverse teams.
Preferred Qualifications

Bachelor’s degree in Nursing, Public Health, Social Work, Psychology, or related field.

ACLS and PALS certifications preferred.

Bilingual (English/Spanish) strongly preferred.

Experience working with underserved populations and health-related grant programs.

Experience in program evaluation, report writing, and stakeholder engagement.
Additional Requirements

Background check, physical exam, and TB test within 1 year.

Immunization compliance (MMR, TDAP, Hep B, Flu, COVID-19) or waivers as applicable.

Willingness to travel for training and periodic meetings across Southern California.
