Registered Nurse (RN) – Indio, CA

Salary: $110,000 – $130,000 (Based on Experience)

Schedule: Mon 8:30am–5:00pm | Tue 10:00am–7:00pm | Thu–Fri 8:30am–5:00pm | Sat 6:30am–3:00pm

Location: Indio, CA | Travel Required (Southern California)

Our client, a leading network of community-based clinics across Southern California, provides integrated medical, dental, mental health, and substance use services. We are currently seeking a mission-driven Registered Nurse (RN) to support their newly launched clinic in Indio, CA.

This is a rewarding opportunity for a dedicated RN who is passionate about community health and public service. You’ll play a leadership role, managing a multidisciplinary team of LVNs and Lead Care Managers, helping shape and expand the delivery of essential health programs such as CalAIM Enhanced Care Management and Community Supports.


Key Responsibilities

  • Provide clinical and programmatic leadership to a team of 11+ staff, including oversight of remote clinic operations.

  • Supervise day-to-day program delivery, including outreach, health education, care coordination, and patient advocacy.

  • Lead the planning, development, implementation, and evaluation of care management programs and related services.

  • Coordinate Multidisciplinary Team Meetings and support collaboration across medical providers, housing specialists, and other partners.

  • Oversee documentation and data entry in eClinicalWorks, ensuring accuracy and compliance with contracts and grants.

  • Monitor performance metrics, evaluate outcomes, and ensure adherence to quality standards.

  • Develop MOUs and maintain partnerships with external agencies.

  • Prepare and deliver internal reports, training sessions, and presentations as needed.

  • Support integration of care management programs into the clinic’s clinical workflow and broader strategic objectives.

  • Participate in relevant local, state, and national conferences.


Required Qualifications

  • Associate Degree in Nursing (ADN) required.

  • Active RN license in California required.

  • Minimum 3–5 years of nursing experience, with at least 1 year in a supervisory or care management role.

  • Current BLS Certification (American Heart Association).

  • Strong working knowledge of health data systems, community health evaluations, and clinical workflows.

  • Excellent communication and leadership skills with demonstrated ability to manage diverse teams.


Preferred Qualifications

  • Bachelor’s degree in Nursing, Public Health, Social Work, Psychology, or related field.

  • ACLS and PALS certifications preferred.

  • Bilingual (English/Spanish) strongly preferred.

  • Experience working with underserved populations and health-related grant programs.

  • Experience in program evaluation, report writing, and stakeholder engagement.


Additional Requirements

  • Background check, physical exam, and TB test within 1 year.

  • Immunization compliance (MMR, TDAP, Hep B, Flu, COVID-19) or waivers as applicable.

  • Willingness to travel for training and periodic meetings across Southern California.
