Contract-to-Hire Case Manager
Location: San Jose, CA (Field-Based + Office Work)
Pay Rate: $27/hour
Schedule: Monday–Friday 8:30am – 5:30pm

Our client is a Federally Qualified Health Center (FQHC) that has proudly served Santa Clara’s diverse and underserved communities for over 50 years. We are seeking a passionate and community-oriented Case Manager to join their dedicated team on a contract-to-hire basis. This role is ideal for someone who thrives in both field and office settings, values collaboration, and is deeply committed to improving the lives of individuals facing complex health and social challenges.

Position Summary

The Case Manager plays a critical role in engaging high-risk Medi-Cal members—including individuals with complex medical, behavioral health, and housing needs. Through outreach, in-home assessments, and care coordination, the Case Manager helps clients access services that promote recovery, wellness, and self-sufficiency.

You will conduct client visits in the community, including homes, shelters, clinics, and partner agency locations. A valid driver's license and reliable transportation are required.

Key Responsibilities

  • Conduct outreach to identify and enroll eligible clients into the CalAIM Enhanced Care Management (ECM) program.
  • Complete comprehensive behavioral health and housing assessments to determine service needs.
  • Develop and maintain individualized care plans in collaboration with clients and care teams.
  • Coordinate services with multidisciplinary providers, including medical, mental health, housing, and social service agencies.
  • Facilitate access to community resources, housing support, and wellness services.
  • Build strong partnerships with local agencies and advocate for client needs.
  • Maintain accurate and timely documentation in compliance with program standards.
  • Provide translation and language support as needed (if applicable).
  • Participate in supervision, team meetings, training sessions, and continuous quality improvement efforts.
  • Perform other related duties and projects as assigned.

Qualifications

Required:

  • Bachelor’s degree in a healthcare, social work, behavioral health, or related field, OR equivalent training and experience.
  • At least 1 year of case management or direct service experience.
  • Familiarity with housing resources and social services in Santa Clara County.
  • Excellent communication, problem-solving, and organizational skills.
  • Cultural competence and sensitivity to diverse populations and family dynamics.
  • Ability to work collaboratively as part of a multidisciplinary team.
  • Valid California driver’s license, auto insurance, and a clean driving record.

Preferred:

  • Fluency in English and one of Santa Clara County’s threshold languages: Spanish, Tagalog, Vietnamese, Mandarin, or Cantonese.
  • Training in trauma-informed care.
  • Experience working with clients experiencing homelessness, mental illness, or chronic illness.
