Contract-to-Hire Patient Engagement and Enrollment Coordinator
Location: San Jose, CA (Field-Based + Office Work)
Pay Rate: $29/hour
Schedule: Monday–Friday 8:30am – 5:30pm

Our client is a Federally Qualified Health Center (FQHC) that has proudly served Santa Clara’s diverse and underserved communities for over 50 years. We are seeking a passionate and community-oriented Patient Engagement and Enrollment Coordinator to join their dedicated team on a contract-to-hire basis. The Patient Engagement and Enrollment Coordinator plays a key role in expanding access to healthcare services by leading both internal and external outreach initiatives. This position is responsible for connecting underserved community residents with our client’s health center services and helping eligible individuals navigate health insurance enrollment. The Coordinator will work closely with community partners and prospective patients to drive awareness, promote preventive care, and increase overall patient enrollment.

Key Responsibilities:
  • Develop and execute outreach strategies to raise awareness of the health center’s services among community members, especially those lacking access to healthcare and insurance.
  • Build partnerships with local organizations to promote insurance coverage options, increase enrollment, and establish effective referral networks.
  • Create and distribute outreach materials, including educational brochures, presentations, and visual displays tailored to diverse audiences.
  • Engage with potential patients to assess needs and assist eligible individuals in enrolling in appropriate health insurance programs.
  • Conduct follow-up outreach to existing patients to close care gaps and encourage participation in preventive health services.
  • Design and deliver training programs for community health workers and outreach staff to support engagement and enrollment efforts.
  • Establish relationships with community influencers (e.g., Promotoras) to enhance visibility and reach within target populations.
  • Participate in media and public relations events to promote services and highlight community impact.
  • Support additional projects and perform related duties as assigned.

Qualifications:
Education & Experience:
  • High school diploma or equivalent; a combination of relevant education and experience may be considered.
  • Minimum of two (2) years of experience in community outreach, enrollment navigation, public health, or community engagement roles.
  • Experience facilitating public events, forums, or informational sessions.
Knowledge, Skills, & Abilities:
  • Strong verbal and written communication skills, with the ability to develop and deliver clear, culturally sensitive presentations.
  • Familiarity with healthcare coverage options, including Medi-Cal, Covered California, and PCAP (Santa Clara County) is preferred.
  • Proficiency in English (verbal and written) is required; bilingual skills in Spanish, Mandarin, Vietnamese, or other languages are highly desirable.
  • Excellent interpersonal skills and the ability to work effectively with diverse populations and multidisciplinary teams.
  • Valid California Driver’s License, current auto insurance, and a driving record that meets agency requirements (position requires regular driving).

Working Conditions:
  • This position involves frequent travel within the community and occasional office-based work.
  • Some evenings and weekend hours may be required to accommodate outreach events and community needs.
  • Use of personal vehicle for travel is essential; mileage reimbursement may be available per policy.
  • May occasionally require use of personal protective equipment (PPE) and adherence to basic safety protocols.

  • Ability to talk, hear, and see to perform duties effectively.
  • Frequent sitting, computer use, and manual tasks involving hands and arms (e.g., typing, reaching).
