ECM Outreach Worker (CalAIM Outreach Coordinator)
Employment Type: Contract to Hire
Pay Rate:  $28
Location: San Jose and adjacent cities in Santa Clara County.
The ECM Outreach Worker (CalAIM Outreach Coordinator) is responsible for conducting community outreach and enrolling eligible clients into the Enhanced Care Management (ECM) program. This role works closely with a multidisciplinary team and community partners to connect individuals to essential services and support systems.

Key Responsibilities
  • Conduct community outreach and engage individuals in various settings to promote ECM services
  • Enroll clients into the ECM program and complete intake assessments
  • Provide case management services and coordinate care for clients
  • Facilitate referrals to community resources and support services
  • Assist clients in developing social supports and community connections
  • Build and maintain strong relationships with community agencies to advocate for clients and increase referrals
  • Ensure timely and accurate documentation in compliance with Medi-Cal, Santa Clara County, and organizational standards
  • Participate in team meetings, consultations, and training sessions
  • Perform additional duties and special projects as assigned

Qualifications
Education & Experience
  • Bachelor’s degree in Public Health, Social Work, or a related field, or equivalent combination of education and experience
  • At least 1 year of case management or related experience
  • Experience with Enhanced Care Management (ECM) and/or Community Supports preferred
  • Experience working with unhoused populations preferred

Knowledge, Skills, & Abilities
  • Knowledge of community and county resources for effective referrals and care coordination
  • Strong interpersonal and communication skills, with the ability to work with diverse populations
  • Ability to build and maintain collaborative relationships within a multidisciplinary team
  • Proficiency with basic technology (computers, email, internet, phones)
  • Working knowledge of Microsoft Office Suite
  • Bilingual skills (Spanish, Mandarin, Vietnamese) strongly preferred

Additional Requirements
  • Valid California driver’s license
  • Current personal auto insurance and acceptable driving record (MVR)
  • Ability to travel locally (up to 25%)
