Community Health Nurse – Black Maternal & Infant Health (Oakland, CA)

AlliedUP is hiring a Community Health Nurse who is deeply committed to improving Black maternal and infant health outcomes in Oakland. This role is ideal for a nurse who understands Oakland’s Black communities, values relationship-based care, and believes health equity requires cultural humility, trust, and community partnership.

About the Role

The Community Health Nurse will provide culturally affirming, community-based clinical support to Black pregnant and postpartum individuals and their families—primarily in West and East Oakland. You will deliver health education, conduct home and community visits, coordinate care, and partner closely with local organizations to address both clinical needs and the broader social conditions impacting birth outcomes.

This role blends clinical expertise with community engagement and is central to AlliedUP’s mission to advance equity in maternal and infant health.

Environment: Community Health
Salary: $146,000 – $180,000 annually

What You’ll Do

  • Provide prenatal, postpartum, and infant health education using a trauma-informed, life-course approach

  • Conduct home and community-based visits, including screenings (e.g., postpartum depression, home safety)

  • Support participants with care coordination and navigation of healthcare and social service systems

  • Build trusting relationships with Black families through culturally affirming, strength-based care

  • Collaborate with Oakland-based clinics, doulas, midwives, and community organizations

  • Participate in group programming and serve as a clinical resource during workshops and series

  • Document services accurately and maintain HIPAA compliance

Who We’re Looking For

  • BSN from an accredited institution

  • Active California RN license; PHN certification preferred or eligible

  • At least 1 year of experience in maternal, infant, or child health (community-based experience strongly preferred)

  • Demonstrated understanding of Oakland’s Black community, including the impacts of structural racism, displacement, and racialized stress

  • Experience serving Black birthing and parenting people with culturally responsive care

  • Strong communication skills and comfort working independently in community settings
