Community Health Advocate / Community Outreach Liaison (Contract to Hire)

Location: Oakland, CA (serving West Oakland, Emeryville, North Oakland, South Berkeley)
Pay rate: $26 per hour
Schedule: Weekdays, Evenings, Weekends as needed.

AlliedUP is partnering with a Community Health Center in Oakland to hire a Contract to Hire Community Health Advocate / Community Outreach Liaison to support Alameda County’s Black Infant Health (BIH) Program. This is a Contract-to-Hire opportunity with potential to convert to a permanent role.

What You’ll Do

  • Recruit and engage Black birthing and parenting individuals into the BIH Program

  • Conduct community outreach through events, partnerships, referrals, and media efforts

  • Build trust and provide culturally affirming support during pregnancy and early parenting

  • Connect participants to healthcare, social services, and community resources

  • Support care coordination as part of an integrated healthcare team

  • Enter and maintain accurate program data (ETO, Epic a plus)

  • Participate in BIH trainings, community meetings, and outreach events

What We’re Looking For

  • Bachelor’s degree in Social Work, Public Health, Nursing, Education, or related field

  • 1–3 years of experience in community-based or public health settings

  • Lived understanding of African-American culture and community health issues

  • Strong outreach, advocacy, and relationship-building skills

  • Excellent communication, organization, and follow-up skills

  • Basic computer skills (Excel, Word; Epic experience a plus)

  • Willingness to complete CHW and Certified Enrollment Counselor training

  • Doula certification a plus

Additional Requirements

  • Valid CA driver’s license, auto insurance, and ability to travel throughout Alameda County

  • Ability to work occasional evenings/weekends for outreach
