Registered Nurse (RN) – Contract-to-Hire | Full-Time
Location: San Jose, CA
Pay: $60/hour
Schedule: Monday through Friday Day Shift | Full-Time
Setting: Federally Qualified Health Center (FQHC)
We are seeking an experienced Registered Nurse (RN) with a strong background in primary care to join a mission-driven community health organization serving Santa Clara’s most vulnerable populations. This contract-to-hire role is ideal for an RN who thrives in an outpatient setting, providing hands-on clinical care while supporting integrated, team-based healthcare.
In this role, you’ll combine your primary care expertise with leadership responsibilities—supervising ancillary staff, mentoring nursing students, and ensuring high-quality, patient-centered care.
Key Responsibilities

Primary Care Triage: Assess walk-in patients and phone calls; provide safe, accurate clinical advice.

Clinical Procedures in Outpatient Care: Administer vaccines, perform venipuncture, wound care, and point-of-care testing.

Care Coordination: Review labs and imaging; coordinate timely follow-up with providers to support continuity of care.

Team Leadership: Supervise medical assistants and mentor nursing students in a primary care environment.

Patient Education & Quality: Deliver education that empowers patients to manage chronic and preventive care needs; contribute to quality initiatives.

Documentation: Ensure accurate, timely records in the EHR (EPIC preferred).
Qualifications

Active California RN license (required)

2+ years of RN experience in primary care or outpatient clinic settings (required)

CPR certification (required); ACLS/PALS (preferred)

Demonstrated ability to manage a busy primary care patient panel

Strong clinical judgment, communication skills, and cultural sensitivity

Bilingual skills (Spanish, Mandarin, or Vietnamese) highly valued
